IMSE Key/Building Access Request – Department of Industrial and Manufacturing Systems Engineering

INSTRUCTIONS:

Complete all applicable fields and submit. Requests from students will route to their faculty supervisor first; faculty/staff requests will be processed directly.

The key holder will be notified by email when the request has been approved and a paper form is ready to be picked up from 3004 Black. That form will need to be taken to 108 General Services Building to obtain the key.

KEY RETURNS/LOST KEYS:

Keys must be returned by the keyholder to Building Security Services at 108 General Services Building.

All lost keys are assessed a replacement fee of $30 per key. Overdue keys will be considered lost, and the key holder will be charged the applicable replacement fee.
